Health plans and accountable care organizations (ACOs) have a wealth of information on large groups of people, but don’t have an easy way to share relevant data with physicians and other care providers. Collective Medical’s enhanced functionality, Flags, now enables health plans and ACOs to create notes for large subsets of members and automatically share that information across Collective’s network.
This first-to-market functionality fuels direct, real-time collaboration with a member’s care team, without the need for manual data entry for individual members. Here are three examples of the ways that a health plan or ACO might take advantage of this new feature.
1. “Triple Threat” Guidelines
“Triple Threat” medications—opioids, benzodiazepines, and muscle relaxants—put an individual at a high risk for harmful, or even fatal, drug interactions when prescribed with other medications. Flags enables health plans to tag all members on these medications, which then helps clinicians make informed care decisions.
2. Programs and Resources
Health plans and ACOs often provide at-risk patients with intensive case management resources, a dedicated care manager, or enrollment in a specific program. Busy care teams armed with this knowledge can help direct a patient towards more appropriate care settings and support patients in taking advantage of the resources provided.

Providing physicians, nurses, case managers, and social workers with relevant and valuable information allows care teams, health plans, and ACOs to work together to provide the best and most comprehensive care.
